Student Forms

Any student enrolled in classes or activities at The Country Schoolhouse must have all three forms on file prior to class attendance.

Medical Information Form and Policies & Procedures Form must be completed online. The Waiver of Liability Form must be turned-in as a hard copy. You may print your own copy to complete and return or attain a hardcopy from The Country Schoolhouse office.

Tuition Payment Information

Full Tuition Payment Option

Payment for entire semester is paid upfront, before the start of classes.

  • When registering student for classes, full tuition payment option will be processed at the end of registration form to be able to complete submission.

Installment Tuition
Payments Option

We are happy to be able to offer an installment payment plan for our semester tuitions. Please read the information below to learn about criteria for this payment plan.

  • Once you have made a payment on your first installment invoice, your payment method is saved to your account. We no longer require a hard copy form and keep no CC or bank info on file at our facility. All of our payment information is securely stored with our payment processor company. We do not have access to your complete account numbers, only the type of card and last four digits of the card #.

  • Installment payments are due on the 5th of each month. An invoice is auto generated on the 1st of each month and sent via email for your review and as a reminder of the upcoming payment. If you would like to use an alternative form of payment, other than what is already saved for your account, you may make payment manually through your invoice email, otherwise, your invoice will be auto-drafted on the 5th with the payment method on file.

  • If you prefer to pay in cash or check, you must pay for the full semester tuition or the current remaining balance on your account. If you elect to continue the installment payment plan, a CC must be entered in order to secure payments for the remainder of the semester. This is a protection for our teacher contractors. The products offered at our facility are service hours. Once those service hours have been rendered, the teacher has no recourse to refund those hours.
